Annual enrollment includes:
3 periodontal maintenance cleanings
2 exams
1 emergency exam
X-rays and topical fluoride (as needed)
30% off scaling & root planing (SRP)
20% off all other treatments
*For patients aged 13 or older with ongoing perio maintenance due to periodontal disease.
The Not-So-Fine Print
Exclusions & Benefits:
- No annual maximums, waiting periods, or deductibles — just straightforward savings.
- Cosmetic teeth whitening and orthodontics are excluded from the 20% discount.
- Orthodontic treatments will receive a $750 discount off the standard fee.
- Payment is required for all treatments at the time of service.
- The membership is non-transferable to other patients, family members, or dental practices.
- Please note this is not an insurance plan and cannot be combined with any existing insurance coverage.
